Insurance plans accepted in 2024:

* Intermountain (out of network referrals with prior authorization ONLY)

* Medicare

* United Health Care Plans including AARP and OPTUM (Dr. Strada is NOT in-network with these plans. Referral may be accepted if the patient has out of network referrals AND with prior authorization ONLY, which must be submitted to the insurance by the referring provider with all the specific codes required for the assessment AND authorized by the insurance)

A referral from your PCP, psychiatrist, or your neurologist is required. The referral must be received with clinical notes, demographics, and copy of insurance card. Please check with your insurance carrier to ensure Dr. Marilyn Strada is in your network. Referrals can be faxed by your physician to 562-478-4575

Plans specifically NOT ACCEPTED in 2024 (Dr. Strada has made efforts in the past to work with the plans listed below, but unfortunately their terms have not been acceptable. Therefore, they are specifically not accepted at this time):

AARP Complete as primary (secondary supplemental is accepted, as primary, the referral must be accompanied by a Pre-Authorization with the specific CPT codes)
