* Intermountain (out of network referrals with prior authorization ONLY)

* United Health Care - AARP, Medicare Advantage Plans (out of network referrals with prior authorization ONLY, not all UHC plans are accepted)
A referral from your PCP, psychiatrist, or your neurologist is required. The referral must be received with clinical notes, demographics, and copy of insurance card. Please check with your insurance carrier to ensure Dr. Marilyn Strada is in your network. Referrals can be faxed by your physician to 562-478-4575
Plans specifically NOT ACCEPTED in 2026 (Dr. Strada has made efforts in the past to work with the plans listed below, but unfortunately their terms have not been acceptable. Therefore, they are specifically not accepted at this time):
